You can download the new patient registration form here, fill it out at home, and bring it with you. If you’d prefer, you can also email them to our secure submission address before your visit. Preparing them in advance makes your child’s check-in quick and stress-free.
Parents can submit forms in one of two ways: by emailing them to our dedicated, secure submission email or by printing them and bringing them to the appointment. We recommend sending them ahead of time whenever possible so we can review them before your visit.
If you choose to email your forms, please make sure they are sent from a private, secure account. You should include the patients’ name and DOB in the email subject line. This is not a monitored email, so you should call the office during business hours to verify that your form was received. Note that fees will apply to forms requested by email.
Yes, and that’s because medical forms contain sensitive health information, we follow strict HIPAA regulations to keep your child’s data private. Email submissions should always be sent to our secure, designated address, not to a general inbox or staff member’s personal email.
We do not recommend sending forms through unsecured public networks, like public Wi-Fi, to help protect your information. If you have concerns about privacy, you can always bring forms directly to our office instead of sending them electronically.
