We will not hold Saturday Clinic Hours from May 23 through July 4 (Memorial Day weekend through July 4th weekend). The on call physician will be available to answer questions between 8am and 11am on each of these Saturdays. Please call the office at 205-879-7888 to reach the answering service. Calls outside of 8am and 11am on these Saturday dates will follow our usual after hours call protocol (after hour fees will apply when requesting to be connected to a nurse). Saturday clinics will resume on Saturday, July 11.
WHEN SUBMITTING AHSAA Sports Physical form requests, a completed and signed copy of page 1 of this form must be uploaded with the form request.
When uploading camp forms (or other activity forms), these will be ready for pickup at reception within two business days (unless the email address is indicated on the form – we will only send completed forms directly to the parent/guardian/patient).
The fee for after hour calls has increased to $25* and the fee for all forms (including Immunization Forms) has increased to $15.
*When requesting to be connected to a nurse after hours, this is considered an after hour call (you are being connected to the Children’s of Alabama Triage Nursing Staff).
