All professional services provided by Alabama Pediatrics are charged to the patient. We file to most major insurance companies except for Medicaid and Medicare. Please contact your insurance company to confirm that we participate in your plan. It is the responsibility of the patient/guarantor to make sure we have the correct insurance information on file. We will be happy to file your insurance for you, but the patient/guarantor is responsible for all fees regardless of insurance coverage. Because there may be multiple insurance policies on a patient, we adhere to the Alabama state birthday rule for the insured. It states, "When there is dual coverage on a child, whichever parent's birthday falls first in the year is the primary insurance". In cases of divorce it will depend on the court's ruling to determine who is responsible for maintaining insurance coverage on the patient.
Copays are due and payable at the time of service. A $10.00 fee will be added to all copays not paid at the time of service. Most insurance plans don't require copays for preventive visits; however, there may be some routine services performed that aren't covered by your insurance plan. If you have a balance due on your account, you will be asked to pay at the time of service. If you can't pay we will be happy to make payment arrangements.
We accept cash, check, and credit/debit cards (Visa, Mastercard and Discover.) You may pay via the payment portal on our website, by telephone, in person, or by mail. There is a $30.00 charge on all returned checks.
A statement will be sent at the beginning of each month. We expect all balances to be paid in full. If payment arrangements are not made and the account is delinquent for more than 90 days it will be sent to collections. The patient/guarantor will be responsible for all costs incurred.
There is a fee as well as a 24 hour waiting period for all medical forms not requested at the time of service. Immunization forms, camp forms, and sports physical forms not requested at the time of service will be $5.00 each. The only exception to this rule is for Family Medical Leave paperwork. FMLA papers will be $35.00 and will have a waiting period of 5-7 days for completion.
